Dear Parent/Guardians of Sixth Grade Students,

New Illinois state law requires all incoming 6th graders to have a current dental health exam on file with the school's Health Office by May 15, 2009. The results of the dental health exam must be completed, dated and signed by a licensed dentist. The State is requesting that only the new version of the Illinois Department of Public Health "Proof of School Dental Exam" form be used.

For those parents/guardians that may require a waiver for this dental exam, only the Illinois Department of Public Health's "Dental Exam Waiver" form should be used. The completed waiver must also be returned to the school health office by May 15, 2009.
