Dear Parent/Guardians of Sixth Grade Students,

Reminder about state physical exam requirements:
Parents/guardians of incoming 6th grade students The state required physical exam and an immunization record must be on file with the middle school health office on or before the first day of school, Tuesday, August 26, 2008. The exam must be dated within one year from the school start date. EXCEPTION FOR THIS YEAR ONLY: The requirement will be waived for any student who submitted a physical exam in 5th grade. All parents/guardians of current 5th grade students will receive a
letter from the elementary school nurse indicating what is needed.
