The Special Education Department in Oak Park Elementary District 97 is attempting to gather meaningful information aimed at helping us improve our delivery of special education and related services. We are asking that, at the end of each IEP meeting, parents/guardians complete a survey that will give us information about how well we are doing things here in District 97. In many instances, the surveys are being sent out with the notice of the IEP meeting.If the parent/guardian didn't receive a copy of the survey prior to the IEP meeting, at the end of the meeting the case manager, team facilitator, or coordinator should offer to provide a copy of the survey along with a pre-addressed envelope. Parents/guardians have the choice of completing the survey before they leave the IEP meeting, in which case they could place it in the envelope and ask that the school representative send it to the Department of Special Education via school mail. It could also be sent in via post mail. Or, the parent/guardian can finish the survey at a later time and then, either drop it in the mail or bring it to the District 97 Administrative Center at 970 Madison in Oak Park.View a copy of the PARENT/GUARDIAN SURVEY
