If Your Child has Asthma:

Managing a child’s asthma is a team effort, involving you, your child, your child’s healthcare providers, and the school. All members of the team want to be able to communicate, ask questions, and share information about your child’s health and well-being.  We want to ensure your child has immediate access to their quick relief medication (inhaler).  Please have your doctor complete the District 97 District 97 Medication Authorization Form (form can be found here), a parent/guardian must sign the bottom of the Medication Form, and have your doctor complete an Asthma Action Plan (form can be found here).  If the physican approves the self-carry of an inhaler, the District 97 Self-Carry and Self-Administration of EpiPen and/or Inhaler Form (form can be found here) must be completed and submited to the School Nurse.

YOUR PHYSICIAN MUST RENEW ALL FORMS AT THE BEGINNING OF EVERY SCHOOL YEAR.
