Medication Policy

Administration of Medications

Students may not possess or consume any medications, including over-the-counter medication (this includes Ibuprofen/Advil, Tylenol, vitamins, or cough drops), without the written orders of a physician along with a parent/guardian signature authorizing the administration of the medication at school. 

Parents should make every effort to give prescribed doses of medication at home.

If Your Child Requires Medication at School:  

Parents/Guardians must have their doctor complete a District 97 Medication Authorization Form (form can be found here) and a parent/guardian must sign the bottom of the form.  Medications must be brought to the health office by the parent/guardian in the original pharmacy prescription labeled container or a sealed as purchased over-the counter container.  Expired medications cannot be given at school. 

YOUR PHYSICIAN MUST RENEW ALL MEDICATION FORMS AT THE BEGINNING OF EVERY SCHOOL YEAR.
