A Note From Nurse Kimmy:
Longfellow Families,
I wanted to send out a friendly reminder about the medication policy for District 97. Students may not possess or consume any medication, including over-the-counter medication (this includes cough drops), without the written orders of a physician along with a parent/guardian signature authorizing the administration of the medication at school. Parents should make every effort to give prescribed doses of medication at home. A parent/guardian must sign the bottom of the form. You can locate this form on the District 97 website (http://www.op97.org/health-services/forms). I also included it as an attachment.
Medications must be brought to the nurse in the original pharmacy prescription labeled container or a sealed as purchased over the counter container. Expired medications cannot be given at school. These forms must be updated annually. Medications without an updated form cannot be administered in the school.
You can fax this form, Attn: School Nurse at 708- 524-3037, scan and e-mail the form, and/or turn the form into the health office.
Emergency action plans (seizure action plan, asthma action plan, food allergy action plan, etc.) need to be updated annually as well. You can find these forms on the district 97 website (http://www.op97.org/health-services/forms).

Bear Necessities Donations: At Longfellow, out students can earn Bear Necessities for demonstrating respectful, responsible, and safe behavior. Each week, students may turn in their Bear Necessities to participate in a raffle. They may also purchase items weekly with their Bear Necessities from the Bear's Den.
