Student Health Requirements

PLEASE NOTE: Deadline for submitting your physical examination and immunizations along with Vision exam (for KG or First Graders) is October 15, 2020. Please submit the physical and immunizations by that date to avoid exclusion from attending school. I realize we are beginning the school year remotely, but those deadlines are still in place.

Additionally, please feel free to scan and email those documents to me at my email address: [email protected]

Please do make copies of forms for yourself, if you have somehow provided to district office or have stopped by the school to drop those forms off.

Please be sure the forms are dated and signed and that the parent portion of page 2 of the physical has been completed by you as the guardian/parent.
