Deadline for School Health Requirements: Kindergartners, Sixth-Graders, and New Students
The State of Illinois requires all kindergartners, sixth-grade students, and students new to the district to submit a current school physical and up-to-date immunization records by the end of the day Oct. 14. Detailed information is available under “Health Requirements.”
Under Section 27-8.1 of the Illinois School Code, District 97 must exclude students who have not submitted the required documentation by Oct. 15. Students who register after Oct. 15 must provide these records within 30 days of enrollment. Students will not be allowed to attend school until all required forms are received.
If your school nurse has contacted you about missing documentation, please submit the required records directly to them before the deadline. Thank you for your attention to this important requirement.
