D97 Survey: Social-Emotional and Mental Health Needs

Dear District 97 Staff and Families,

Oak Park Elementary School District 97 is continuing to plan for the 2020-21 school year and is prioritizing the social-emotional and mental health needs of students, staff, and families to ensure the availability and quality of appropriate resources and supports. We have partnered with Hanover Research to design and analyze a survey to understand your needs and priorities related to social-emotional and mental health for this school year, and we would like to hear from you.

You can access the survey here or by copying and pasting the following URL into your browser: https://hanover-research.qualtrics.com/jfe/form/SV_39RQ56KYF6mBgwZ.

The survey will close on Monday, Aug. 24.

Please note that your responses to this survey will be completely anonymous and will only be shared with District 97 in aggregate form, so please be candid in your responses.

We appreciate you taking the time to complete this survey.

Sincerely,

Dr. Carrie Kamm
