Kindergarten and New Student Registration for the 2016-17 School Year

Requirements for Registration

  • Only individuals who are current Oak Park residents can register a child in District 97.
     
  • A child must be five years old on or before September 1, 2016 (i.e., born on or before September 1, 2011) to be eligible for enrollment in kindergarten.
     
  • Parents/guardians of students entering kindergarten are encouraged to complete the registration process prior to the Kindergarten Open House, which will be held at each of our eight elementary schools on May 4, 2016.

Steps for Registration

  • Please read through all of this information.
     
  • Schedule an appointment with the Registrar’s Office to provide the required documents and start the registration process. There is a link at the bottom of this page you need to use to schedule an appointment online.Appointments can only be made/accepted via the online scheduler
     
  • Provide all required documents at the time of the appointment.Partial documentation will not be accepted. If parents/guardians are unable to attend the appointment, a family representative (e.g., childcare provider, relative, etc.) may come in their place as long as they bring originals of all of the required documents.
     
  • At the time of your appointment, your documentation will be reviewed for approval. Once approved, you will be issued a “Snapcode” that will give you access to Infosnap, which is the district’s online registration site.
     
  • You can use a personal device to complete the online registration at a time and place that is most convenient for you. If you do not have a personal device, the district will provide you with access to a computer you can use to finish the registration process. We encourage you to complete this process at your earliest convenience.Your student will not be enrolled in the district until his/her online registration has been submitted.
     
  • Once your online registration has been submitted, you will receive a confirmation email from Infosnap.

Documentation for Registration

The residency status for all students will be established in accordance withpolicy 7:60 (Residence)

The following documents are required when registering a student for school in District 97:

  • Student’s official birth certificate
  • Parent/guardian picture ID
  • Current physical/immunization record on theState of Illinois Certificate of Child Health Examination form
  • Illinois State “Good Standing” transfer form (only required of students transferring from an Illinois public school, grades one through eight)
  • Custody documents (if applicable): court order, agreement, judgment or decree that gives custody of the child to any person

*All students must provide their most recent physical/immunization record at the time of registration. For students entering kindergarten and early childhood programs, if this record is not dated within one year prior to the first day of school, the parent/guardian is required to present an updated exam to the school nurse by the student's first day of attendance in the district.

AND any ONE of the following:

Plus any THREE of the following that must show the current Oak Park address:

  • Illinois vehicle registration
  • Illinois driver’s license/ Illinois State ID
  • Oak Park vehicle sticker receipt
  • Voter registration card
  • Most recent cable or credit card bill
  • Current public aid/medical card
  • Current proof of homeowners/renters insurance
  • Most recent utility bill (gas, electric, or water)
  • Most recent bank statement
  • Piece of U.S. mail received at residence
  • Receipt from moving company or moving van rental
  • Current Oak Park library card
