Julian Athletics
Sports Physicals
Please remember that every student must have a current sports physical in order to participate in athletics at Julian.
6th Grade
Students may submit either a separate sports physical or their school physical as long as the sports clearance box is checked and signed by their healthcare provider.
7th and 8th Grade:
Students must submit a separate sports physical every year. Please note: Sports physicals expire 13 months (395 days) from the date of the exam.
We ask that families provide two copies of the completed physical before tryouts or practices begin:
- One copy to be given directly to the coach
- One copy for the Health Office
If these requirements are not met, students will not be permitted to try out for their team.
